As a Licensed Practical Nurse (LPN) with our organization, you will have the opportunity to deliver exceptional care to our residents while collaborating with a multidisciplinary team of healthcare professionals.

About the Role:

As an LPN at Civita Care Center at Newington, you will be responsible for providing high-quality patient care to our residents, under the supervision of our licensed nurses and physicians. You will be expected to:

• Assess patient needs and develop personalized care plans that address their unique requirements

• Administer medications and treatments as prescribed by physicians

• Provide wound care and maintenance of complex medical equipment

• Conduct vital sign checks and report any changes or concerns to the nursing staff

• Participate in quality improvement initiatives to ensure the highest standards of patient care

Responsibilities:

• Collaborate with the interdisciplinary team to develop and implement individualized care plans
• Provide direct patient care, including but not limited to:
+ Vital sign checks
+ Medication administration
+ Wound care
+ Medical equipment maintenance
• Maintain accurate and detailed records of patient care and progress
• Participate in quality improvement initiatives and contribute to the development of policies and procedures
• Demonstrate excellent communication and interpersonal skills when interacting with patients, families, and healthcare staff
