Patient Care Manager / Registered Nurse, Home Health

Role Overview The Patient Care Manager supports patients and clinical staff in delivering compassionate, high-quality home health services. They coordinate and monitor patient care, supervise clinical personnel, and ensure compliance with professional standards and regulations. The role involves fostering a culture of safety, excellence, and teamwork, influencing quality outcomes, and promoting staff development through coaching and ongoing education. Qualifications Licensed registered nurse with current licensure in the practicing state. Prior home care experience with at least two years in management or supervision is preferred. Knowledge of professional standards, Medicare Conditions of Participation, and regulatory requirements is essential. Key Responsibilities Provide clinical supervision, coordinate patient care, supervise staff, maintain effective communication with patients and referral sources, support quality improvement initiatives, uphold compliance, promote safety, and encourage staff development.
