MSW: Geriatrics Care & Coordination (Onsite)

Pacesemi is seeking a Master’s Social Worker to work independently within the Interdisciplinary Team in Center Line, MI. The MSW assesses needs, coordinates services, and delivers care using bio-psychosocial assessments, CBT interventions, and care planning. You will develop treatment plans, coordinate with the team, document in the EHR, and advocate for participants.
