Behavioral Health Case Manager — Care Coordination
Detroit Wayne Mental Health Authority
The Detroit Wayne Mental Health Authority is seeking a Case Manager in Detroit to oversee a caseload of members with behavioral health needs. Responsibilities include assessing treatment needs, developing and evaluating treatment plans, and facilitating care coordination. The ideal candidate will have a Bachelor's degree in Social Work or a related field and two years of professional clinical experience in a behavioral healthcare setting. In this role, you will collaborate with various stakeholders to ensure members receive quality care and support.
