Pay: $72,012.00-$80,000.00 per year

The  RN/LPN  is responsible for providing direct nursing care to residents in accordance with federal, state, and local regulations, facility policies, and accepted standards of nursing practice. The nurse will assess resident needs, develop and implement individualized care plans, administer medications and treatments, monitor resident conditions, and collaborate with the interdisciplinary team to ensure the highest quality of care. The Staff Nurse serves as a resident advocate and promotes a safe, compassionate, and therapeutic environment while maintaining compliance with Department of Health regulations and facility standards.

Essential Duties and Responsibilities

Resident Care

- Assess residents upon admission and throughout their stay to identify physical, emotional, psychosocial, and clinical needs.

- Monitor resident conditions and promptly report significant changes to the physician, nursing supervisor, and interdisciplinary team.

- Provide direct nursing care in accordance with established nursing standards and facility policies.

- Ensure residents receive quality care that promotes dignity, independence, and quality of life.

- Respond appropriately to emergencies and changes in resident condition.

Documentation and Compliance

- Maintain accurate, timely, and complete clinical documentation in the electronic medical record (EMR). 

- Document assessments, interventions, resident responses, incidents, and communications with physicians and family members.

- Ensure compliance with CMS, Department of Health, OSHA, HIPAA, and facility regulations.

- Participate in surveys, audits, inspections, and quality improvement initiatives.

- Complete required reports and documentation according to facility policy.

Leadership and Team Collaboration

- Supervise and provide guidance to Certified Nursing Assistants (CNAs) and other direct care staff.

- Delegate nursing tasks appropriately and monitor completion.

- Collaborate with physicians, therapists, social services, dietary services, and other departments to coordinate resident care.

- Participate in shift reports, care conferences, and interdisciplinary meetings.

- Assist in orienting and mentoring new nursing staff as assigned.

Resident and Family Communication

- Communicate effectively with residents and families regarding care plans, treatments, and changes in condition.

- Address resident and family concerns in a professional and timely manner.

- Promote resident rights and advocate for resident-centered care.

Required Qualifications

- Current and active Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensed in the State of New Jersey.

- Graduate of an accredited nursing program.

- Current CPR/BLS certification.

- Knowledge of nursing principles, practices, and standards of care.

- Ability to accurately administer medications and treatments.

- Strong clinical assessment and critical thinking skills.

- Excellent communication and interpersonal skills.

- Ability to maintain confidentiality and comply with HIPAA regulations.

- Ability to work independently and as part of an interdisciplinary team.

Preferred Qualifications

- Long-Term Care, Skilled Nursing Facility, Rehabilitation, Subacute, or Geriatric Nursing experience preferred.

- Experience with Sigma care or other electronic health record systems preferred.

- Wound care certification or experience preferred.

Physical Requirements

- Ability to stand, walk, bend, lift, and move throughout the facility for extended periods.

- Ability to lift, transfer, reposition, and assist residents in accordance with safe patient handling procedures.

- Ability to respond quickly during emergencies.

- Ability to perform repetitive tasks associated with resident care and documentation.

Salary/Compensation: $72,012 - $80,000 per year
