Family Support Navigator: Home Visits & Community Access

Communitas, Inc.

Communitas, Inc. in Wakefield, MA is seeking an Intensive Family Support Coordinator to work with families, conduct home visits, and identify ways to access community supports and DDS services. You will create Family Support Plans that meet individual needs, assist families with SSI, SNAP, MassHealth, housing, and transportation, and collaborate with clinical providers to ensure coordinated care. #J-18808-Ljbffr Communitas, Inc.
