Exciting Opportunity for LPN in Mobile Case Management!

Job Description:


- Position Type: LPN, Full Time
- Shift: Day 5x8-Hour (08:00 - 16:30)
- Location: Mobile position based at Riedman Administrative Campus
- Schedule: Monday to Friday, 8:00 a.m. - 4:30 p.m.
- Specialty Focus: Adult/Geriatric Care

Key Responsibilities:


- Support the Transitions of Care program aimed at reducing 30-day readmission rates.
- Work under the direction of the RN Care Manager to assist in implementing care plans.
- Manage a rolling caseload of approximately 25-30 patients monthly.
- Conduct appointment scheduling and biometric data monitoring.
- Provide and reinforce patient education.
- Attend patient visits and complete home visits as necessary.
- Connect patients with community resources and maintain frequent contact.
- Document clearly and concisely in the Electronic Health Record (EHR).
- Participate in care team rounds to provide patient updates.

Requirements:


- Experience: Minimum of 1 year in Home Health or Mobile Case Management.
- Certifications: BLS Required.
- Skills:
- Strong communication and documentation skills.
- Ability to work independently and remain adaptable throughout the day.
- Clinical knowledge of CHF, COPD, and complex comorbidities preferred.
- Transportation: Access to reliable transportation for travel during the workday.
