Certifications: CPR, BLS
Must-Have: Proficiency in HomeCare HomeBase EMR and OASIS


Description:
Provides and coordinates home nursing care according to physician's orders.
Adheres to agency policies and procedures through the competent application of the nursing process.
Responsible to the Clinical Manager.
Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Home visits and periodic office meetings are required.
Must have reliable transportation, a current driver's license, and agency-required liability insurance.
Average number of patients per day: 5-8.
Contractor will be assigned between 5-10 patients per day.
Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
Weekend requirements may vary based on location.
On-call requirements vary per location and profession.
Territory covers a mileage radius of up to 75 miles.
Mileage will be reimbursed at the IRS standard rate.
Local candidates are accepted.
The contractor must be willing to travel up to 75 miles from their home clinic.
All documentation must be completed within 24 hours of the appointment.
Late or incomplete documentation may result in cancellation of the contract.
There are no guaranteed hours with this contract.
Clinicians must hit weekly productivity expectations.
All overtime must be pre-approved.
If there are not enough patients to support financial needs, there are options to float to another nearby clinic.
Proficiency in HomeCare HomeBase and OASIS within the last 6 months is required.
Must accept all patients on the schedule regardless of distance.
