Benefits Medical, Dental, and Vision coverage starting Day One 3 weeks of Paid Time Off annually 10 paid Holidays annually 403(b) Retirement Savings Plan Tuition Reimbursement for eligible programs Employer‑paid Short Term and Long Term Disability Employee Assistance Program Life insurance Vision insurance Qualifications Bachelor’s degree from an accredited nursing program Registered Nurse license in good standing with the Arizona State Board of Nursing 2–5 years of clinical experience Demonstrated understanding of clinical methods and record-keeping systems Ability to supervise and apply principles of clinical supervision Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and patient management systems Valid Arizona driver’s license Certification in CPR and AED (American Heart Association or Red Cross) Valid Level One Fingerprint Clearance Card from the Arizona Department of Public Safety for behavioral health locations Responsibilities Perform triage calls using clinical decision support tools for adult primary care, OB, and pediatric populations Participate in team, specialty team meetings, and quality and process improvement initiatives Facilitate patient education sessions for adults, pediatric, and OB patients Coordinate continuity of care with external providers, including referrals and two-way communication Develop relationships with external health organizations and resources Coordinate outreach to high-risk patients and oversee hospital discharge and admission processes Collaborate with health plans, hospitals, and skilled nursing facilities for transitional care needs Conduct comprehensive preventive screenings and assist staff during daily patient interactions Promote clear communication within the care team and with treating clinicians regarding patient care plans Implement standing orders and protocols for patient care Work with the clinical team on data collection, health outcomes reporting, audits, and program evaluation Review and act on care team tasks and inbox messages Review and interpret laboratory results Identify barriers to care and social determinants of health Prepare for and assist with patient examinations in the clinic setting Perform medication reconciliation, prescription refills, prior authorizations, and DME management Provide short-term disability support Perform POCT items (flu, strep, UA, HCG testing) during nurse visits Evaluate clinical care utilization and develop new clinical tools, forms, and procedures Manage insulin titration for diabetic patients, including follow-up calls Deliver patient-specific education plans and provide excellent customer service and communication skills Experience: Nursing – 1 year (Preferred) License/Certification: RN License #J-18808-Ljbffr Adelante Healthcare
