Hope Community Services is a leading outpatient behavioral health provider in South Oklahoma City, serving individuals impacted by mental health and substance use disorders. We provide compassionate, trauma-informed care focused on recovery, wellness, and improving quality of life.
We are seeking a Registered Nurse (RN) Nurse Care Manager to support our Urgent Recovery Center (URC), Intensive, Community Living, and Children & Youth programs. This position is ideal for an RN who has strong clinical judgment, enjoys working in a fast-paced environment, and is passionate about providing coordinated, client-centered care. It is a dual position, providing on-site nursing support within the URC while also supporting care coordination across programs.
The RN Nurse Care Manager plays a key role in ensuring safe, high-quality, and compliant nursing care, while supporting integrated, recovery-focused services for the individuals we serve. This position serves as an on-site nursing resource within the URC while maintaining meaningful involvement in care coordination.
AVAILABLE SHIFT
This role follows a four (4) ten-hour shift schedule (WednesdaySaturday, 10:00 a.m.8:00 p.m.) based in the Urgent Recovery Center.
ABOUT THE POSITION
This dual RN role combines direct clinical support within the Urgent Recovery Center (URC) with nurse care management responsibilities across programs. During scheduled shifts, the RN serves as an on-site clinical resource, providing assessment, guidance, and escalation support to promote safe, high-quality care aligned with best practices and regulatory standards.
As part of the URC's integrated care team, the RN works in a 24/7 environment supporting individuals experiencing acute behavioral health crises. This role supports both medical and behavioral health interventions and plays a critical part in ensuring care is safe, timely, and trauma-informed.
In addition to clinical responsibilities within the URC, the RN supports care management activities for Intensive, Community Living, and Children & Youth programs. This includes completing health assessments, coordinating care, and collaborating with interdisciplinary team members, Hope outpatient services, the Medication Clinic, and community healthcare providers.
KEY RESPONSIBILITIES
- Serve as an on-site clinical resource within the Urgent Recovery Center (URC), supporting LPNs and direct care staff
- Perform RN-level assessments and reassessments for individuals whose needs exceed LPN scope of practice
- Provide real-time clinical guidance and delegation to support safe, compliant nursing care
- Support adherence to URC protocols, standing orders, and evidence-based standards of care
- Interpret and apply standardized assessment tools (e.g., RASS, CIWA-AR, CIWA-B, COWS, PANSS-EC, OASIS)
- Facilitate appropriate escalation of medical concerns in collaboration with URC leadership, prescribers, and nursing leadership
- Ensure clinical documentation is accurate, timely, and complete
- Conduct health screenings and nursing assessments for Intensive, Community Living, and Children & Youth program participants
- Coordinate care for individuals with chronic medical conditions and complex health needs
- Provide education related to medications, chronic disease management, and overall health maintenance
- Collaborate with interdisciplinary team members and external healthcare providers
- Support coordination of benefits and linkage to primary care and specialty services
- Maintain accurate clinical records and actively participate in care coordination meetings
REQUIREMENTS
- Current, unencumbered Registered Nurse (RN) license in Oklahoma
- Minimum of 35 years of RN experience (urgent care, emergency, behavioral health, detox, or community-based care strongly preferred)
- Current BLS certification
SKILLS
- Demonstrated ability to function in a charge nurse role, providing clinical direction and real-time guidance to LPNs and direct care staff during assigned shifts
- Strong understanding of RN delegation responsibilities ensuring all care is delivered within the Oklahoma Nurse Practice Act, accepted standards of nursing practice, and agency policy.
- Ability to monitor and evaluate nursing care delivered by LPNs during the shift, identify practice concerns or scope-related issues, and promptly intervene or escalate concerns through established clinical and administrative channels.
- Demonstrated competence in charge-level medication oversight, including participation in medication counts, identification of discrepancies, accurate documentation, and timely escalation with identified discrepancies.
- Strong clinical judgment and prioritization skills, with the ability to manage multiple competing demands, and use sound nursing judgement in a fast-paced, high-acuity environment.
- Ability to recognize changes in patient condition, synthesize assessment data, and initiate appropriate nursing actions or escalation using standing protocols, provider collaboration, and chain of command.
- Effective communication skills to support interdisciplinary collaboration and real-time clinical problem solving.
- Ability to support clients in learning to navigate healthcare systems and actively participate in their own care, with the goal of increasing independence in health maintenance and chronic disease management.
