Registered Nurse - Case Management- Case Manager

Job Details

We are looking for a traveler who can quickly step into a fast-paced acute care environment with minimal onboarding. We would appreciate candidates who can start as soon as possible and function independently in a high-volume acute care setting. Ideal candidates will have:?\tStrong discharge planning and care coordination experience in an inpatient hospital setting.?\tKaiser Permanente experience, particularly in utilization review and discharge planning workflows.?\tFamiliarity with post-acute placement processes, including SNF, home health, DME, transportation, and community resource coordination.?\tExperience collaborating with multidisciplinary teams, physicians, nursing, social work, and external agencies to facilitate timely and safe discharges.?\tStrong Epic documentation and communication skills.?\tAbility to manage a high-volume caseload, prioritize competing demands, and proactively address barriers to discharge.?\tKnowledge of Medicare, Medi-Cal, and managed care authorization processes is highly desirable.?\tProactive, organized, and comfortable working independently while partnering effectively with operational and clinical teams to support patient throughput and safe transitions of care.Previous case management experience and demonstrated experience in utilization management, discharge planning, or transfer coordination.

Required Qualifications

CA RN, Kaiser Exp Required, 2 years recent in-patient Patient Care Coordinator experience. Registered Nurse, Case Manager
