Job Description

We are seeking a dedicated healthcare professional with clinical experience in med-surg and post-acute care settings. The ideal candidate will be a licensed individual, with preference given to social workers. The role requires effective communication skills, basic computer proficiency, and an understanding of reimbursement benefits, including Medicare, Medicaid, and managed care.

Responsibilities

• Provide comprehensive healthcare services in med-surg and post-acute care settings.
• Coordinate patient care and manage case documentation.
• Advise patients on reimbursement benefits and requirements.

Required Experience / Certifications / Licensure

• 3-5 years of clinical experience in healthcare settings.
• Current state license/certification to practice.
• Social Worker certification preferred.
• Knowledge of Medicare, Medicaid, and managed care systems.
