Clinical Pharmacist-Pain Management

Job Summary The Pain Management Clinical Pharmacist will provide direct patient care and pharmaceutical services. The Clinical Pharmacist will maintain comprehensive medication therapy management and services under a defined scope of practice. In addition, will review electronic medical records to monitor drug therapy for appropriateness, and adverse drug reactions. Required Qualifications Education: Doctor of Pharmacy (Pharm.D.) Education: Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE) or a Foreign Pharmacy Graduate Examination Committee (FPGEC) certificate and active licensure from a U.S. state, territory, or district. Licensure: Current, full, active, and unrestricted license to practice as a clinical pharmacist in one of the 50 US States and/or territories Certification: Basic Life Support (BLS) Internship/Residency: Completion of a clinical pharmacy internship, residency and/or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy Additional Pharmacist Privileges: Certification by the Board of Pharmacy Specialties in one or more recognized practice areas appropriate for the privileges requested (e.g., Pharmacotherapy, Ambulatory Care Pharmacy, et.); or completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health-System Pharmacists or American College of Clinical Pharmacy; or completion of a Post-Graduate Master of Science degree with emphasis in medication therapy management or direct patient care. U.S. Citizenship is required Key Responsibilities Assess patient’s response to drug therapy and planning drug therapy based on physician-established diagnoses. Order and evaluate laboratory tests necessary to evaluate drug therapy effects and outcomes. Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states (e.g., hypertension, hyperlipidemia, anticoagulant, diabetes, asthma, refill clinics, etc.), in cooperation with the medical staff. Assess metabolic needs and order therapeutic enteral or parenteral nutrition products in the inpatient setting. Evaluate medical and medication histories for drug-related problems and adjusting drug therapy accordingly. Consult with other healthcare providers (e.g., physicians, dietitians, nurses, physical therapists, etc.) about patient treatment needs or options. Perform a full range of pharmacist procedures. Review patient profiles at the time of dispensing in order to closely monitor medication therapy. Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age specific dosage, and other pertinent data are appropriate for individual patients. Ensure accuracy of all pharmaceutical products prepared or processed by technical staff. Provide medication and healthcare related in service training to clinical staff on new medications on the market; appropriate use of medications; comparison of current medication therapies; and other topics as deemed appropriate by the department head. Consult with other specialty practitioners to provide optimal patient pharmaceutical care. Additional Duties Conduct a clinical interview with patient, family, and/or caregiver(s) to identify information necessary to design a pharmacotherapeutic treatment plan. Order and evaluate appropriate laboratory tests to assess clinical response, development of adverse drug reactions, therapeutic drug levels, and complete all appropriate clinical reminders. Document and communicate all information as appropriate; pain patient agreements, pharmacotherapeutic treatment findings, and recommendations. Educate patients and their families/caretaker(s) on medications and non-drug therapy and provide appropriate written drug information: counsel on self-monitoring techniques and lifestyle changes to promote wellness; emphasize medication adherence and provide compliance aids, such as medication charts and pill boxes. Extend existing prescriptions (psychotropic and non-psychotropic medications) as appropriate and ensure the patient receives follow-up appointment(s) with the appropriate individual(s). Order consults and referrals as appropriate. Participate in an on-site designated Extension for Community Healthcare Outcomes (ECHO) telementoring training program. Work Environment and Schedule On site, Monday- Friday, 0630 -1800 Compensation & Benefits Up to $161,000 per year, depending on experience and qualifications. Physical Requirements Prolonged standing and walking; frequent computer and telephone use; manual dexterity for handling medications and pharmacy equipment; ability to read medication labels and patient records; occasional lifting/carrying up to 25 pounds; and ability to bend, reach, and move throughout the pharmacy/clinical setting while maintaining attention to detail in a fast-paced environment.
