ACT Team Leader - Social Worker or Therapist

Job Description Under administrative direction, provides clinical leadership to the ACT Team in assessment, treatment planning, and general healthcare, medical, and psychosocial approaches. Assertive Community Treatment (ACT) is designed to engage persons with severe mental illness (SMI) whose needs are not met by traditional outpatient services. Services are delivered in the environment where they are needed, with the team going to the person. Responsibilities Direct day‑to‑day clinical operations of the ACT team. Provide direct supervision of team members. Schedule staff work hours to ensure appropriate coverage. Lead daily team and treatment planning meetings. Evaluate the status of all consumers and coordinate treatment to meet their challenging needs. Direct and coordinate consumer admission and assessment processes. Schedule admission interviews. Supervise, direct, and coordinate comprehensive assessment/reassessment of each consumer. Utilize management tools to ensure the team’s philosophy and goals align with the ACT model. Participate in staff recruitment, interviewing, hiring, orientation, and performance planning. Oversee medical record documentation to ensure quality and accuracy. Provide on‑call crisis interventions and backup according to team policy. Engage and motivate team clients in all treatment goals while providing direct service. Manage the ACT team to maintain organization’s required productivity standards. Oversee completion of all required documentation in a timely manner consistent with organization guidelines. Education & Qualifications Master's degree in Social Work, Psychology, Counseling, or a related field required.
