Rehabilitation Nurse

Supports and promotes the mission and values of the organization. Renders professional nursing care to patients within an assigned unit of the hospital and carries out orders placed by the responsible provider. Provides comprehensive clinical documentation, including care plans each shift. Seeks feedback regarding practice and takes appropriate actions. Communicates patient information appropriately for continuity and safe care through bedside shift report, multidisciplinary rounds, and hourly rounding (AIDET). Compliant in achieving unit-specific competencies and requirements. Completes all net learning modules within the identified time frame. Helps to promote a culture of well-being, such as decreasing alarm fatigue. Promotes a positive culture and atmosphere within the department each shift. Provides a safe and secure environment for patients, visitors, and staff by following established procedures and protocols. Identifies and assists in the correction of patient care safety issues. Works within the guidelines of a Just Culture Environment to promote error reporting and the adoption of safe practices. Assures issues are addressed through appropriate use of the chain of command. Participates in daily unit-based safety huddles. Knowledge and understanding of the organization's overall strategic plan. Remains up-to-date with unit-based nurse-sensitive indicators. Actively participates in quality initiatives and performance improvement to improve patient outcomes. Collaborates with nurses in other departments and units to draw upon expertise in other clinical areas and specialties. Seeks opportunities to assist all team members. Develops and implements performance goals based on self-assessment and performance feedback. Assures patient education materials are culturally competent. Serves as a preceptor for new nurses. Participates in survey readiness. Demonstrates practice that is consistent with the tenets of shared governance and the philosophy of the unit and organization. Assists in identifying solutions in response to problems and conflicts. Effectively delegates aspects of care and unit activities to other nursing personnel. Collaborates in planning, supervising, and instructing other nurses and assistants. Works collaboratively with nursing students and instructors. Participates in peer feedback during annual reviews. Demonstrates characteristics that reflect commitment to co-workers. Manages time off per policy. Completes dysrhythmia course prior to completion of orientation, required for all telemetry-based departments. Maintains current nursing licensure and certifications required for the department. Performs other duties consistent with this role. May enter orders for labs, X-rays, EKGs, or other testing as per protocol and document point of care tests after completion of testing.
