SUMMARY OF POSITION
This position provides comprehensive school social work services to parents, students and school staff, addressing barriers that limit a student from receiving full benefit from their educational experience and responds to referrals from school administration, parents, teachers, the Student Support Team, the Special Education Committee and others by providing direct services and by assisting families in accessing appropriate community resources.ESSENTIAL JOB FUNCTIONS
Conduct a family‑centered interview with the student and his/her family to develop a community‑based Plan of Care using the individualized wraparound process and support the child/youth and the family in meeting the needs and objectives of the Plan of Care. Conduct parent interviews to acquire socio‑developmental information as needed. Analyze information gained through record reviews and interviews to determine environmental impacts and appropriate resource needs; integrate gained information into a written report and oral presentation. Enter and use appropriate case‑specific information into the data system. Maintain an ongoing liaison with community agencies and other resources to meet student needs; refer parents and student to agencies when appropriate. Serve on school committees as assigned relating to school health, safety and climate. Maintain appropriate licensure. Coordinate and monitor services between agencies to meet child/youth client and his/her family needs. Develop cooperative working relationships with all members of the Provider Network serving the child/youth client and his/her family, while ensuring adherence to the Plan of Care. Education: Master's degree from a recognized college or university with a major study in social work, sociology, psychology, counseling, nursing or a related field. Experience: Two years of professional, paraprofessional, or parental advocacy/education experience providing direct service to families, children or youth in social work, psychology, human services, counseling, mental health or equivalent. License/Certification: New Hampshire's valid driver's license and/or access to transportation with liability insurance coverage as required by state laws for frequent travel throughout the state.OTHER KNOWLEDGE & SKILLS
Strong verbal, writing and computer skills to work with diverse children, youth, families, co‑workers, cross‑agency stakeholders, and members of the public. Ability to engage individuals from diverse backgrounds and cultures. Bilingual proficiency is desirable. Experience with minority or underserved populations is desirable. Strong knowledge of public benefit, mental health, child welfare and education systems. Commitment to system of care core values. Ability to maintain strictest confidentiality guidelines. Must be organized and able to work well under pressure meeting deadlines with efficiency and consistency while maintaining job requirements. Physical demands of the job are as follows: Sitting – frequent; Standing – occasional; Lifting/Pushing/Pulling 50 lbs – occasional; Stooping – occasional; Bending – occasional; Reaching – frequent. It is also anticipated that stress will be associated with this position primarily due to the demands of this position. It is necessary that the individual have the capacity to remain calm, considerate, and tactful under stressful and unanticipated situations.SPECIAL CONSIDERATIONS
