We are seeking a Certified Occupational Therapy Assistant (COTA) to join our growing home health therapy team. This is a rewarding opportunity to deliver one-on-one patient care in the home setting, helping patients regain independence, improve mobility, and restore daily function after illness, injury, or surgery. If you are passionate about rehabilitation, patient-centered care, and meaningful outcomes, this role is for you. What You’ll Do
Provide occupational therapy treatments in home health settings under the direction of an Occupational Therapist
Support patients with activities of daily living (ADLs) such as dressing, bathing, grooming, and mobility
Assist in improving strength, coordination, balance, and functional independence
Deliver care focused on rehabilitation, recovery, and quality of life improvement
Follow infection control, safety, and universal precautions
Maintain accurate and timely clinical documentation
Communicate effectively with patients, families, and interdisciplinary care teams
What We’re Looking For
Active Certified Occupational Therapy Assistant (COTA) license
Current CPR certification
Valid driver’s license and reliable transportation
Home health or skilled nursing facility experience preferred
Experience with HomeCare HomeBase (HCHB) preferred but not required
Physical Requirements
Ability to perform hands-on patient care in home environments
Must be able to stand, walk, squat, kneel, crawl, and climb as needed
Ability to lift, carry, push, or pull up to 100 lbs

Working Hours To meet the needs of our patients and clients, employee work hours may vary and will be set by each employee’s supervisor, manager, and operating license documentation. Employees should consult with their supervisors regarding their schedules and time recording procedures. Choice Health at Home, the Company, is open Monday through Friday, office hours are 8:00 am to 5:00 pm (CST). Mileage The Company recognizes the use of personal vehicles by personnel for business purposes as part of their employment. The company will reimburse the eligible employee $0.50 per mile to cover personal care use, auto insurance, depreciation, and gas. Each employee who is eligible for mileage reimbursement from the Company will be required to track and report mileage accurately and timely each pay period. Company vehicle eligibility after 90 days (subject to fleet program requirements and availability)
