Dates of Coverage: As soon as credentialed; ongoing needs to support two full-time providers. Dates will vary. Reason for Coverage: Cover our two Hampton Roads clinics when providers need time off or while one of our positions is vacant. Practice Setting: Occupational Health Schedule: Monday-Friday 8:00-close (close at 4:30pm but can be delayed by walk in injuries) Average Number of Patients per Day: 8-10 Documentation System/EMR: Systoc Number of Exam Rooms: 5 Number of Physicians in Practice: 1 Description of Support Staff: RNs, RTs, MAs, LPN, RN Supervisor, Office Manager, PSRs Submission Requirements

- Board Certified - REQUIRED
- Active VA - REQUIRED
- BLS/ACLS Certified - REQUIRED
- DOT Certification - REQUIRED
- MUST BE WILLING TO COVER LOCATIONS AS LISTED IN JOB
- Immunization - Influenza - Current Flu Season; COVID; TB / QuantFERON TB Gold; MMR - REQUIRED TO NOTE CONFIRMATION AT TIME OF NAME CLEAR
