A skilled Registered Nurse with home health expertise is needed for a 13-week full-time contract. This role is ideal for clinicians with strong OASIS experience and effective documentation skills who are ready to manage a consistent schedule and deliver compassionate, high-quality care to home health patients.

Schedule:

• Full-time commitment
• Monday through Friday
• On-call may be required

Key Responsibilities:

• Deliver skilled in-home nursing care for adult and geriatric patients
• Complete patient assessments and ensure timely, accurate OASIS documentation
• Coordinate with physicians, therapists, and the care team to develop individualized plans of care
• Administer wound care, manage medications, provide disease education, and monitor patient status
• Communicate patient status, progress, and any changes promptly to the clinical care team

Qualifications:

• Active license as a Registered Nurse (RN)
• Minimum of 1 year of recent experience in home health nursing (required)
• Solid proficiency with OASIS documentation (required)
• Experience using HomeCare HomeBase (highly preferred)
• Able to handle daily structured patient visits and a full-time schedule
• Valid drivers license and reliable transportation
