Care Manager - Outagamie

Community Care is hiring a Case Manager/Care Manager to join our Family Care Program team. Community Care’s mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. Position based in Outagamie county.

Care Manager/Case Manager Job Responsibilities:

• Manages an assigned caseload of program members in collaboration with an RN Care Manager
• Completes comprehensive member assessments, member centered care plans, & functional screens
• Provides ongoing care coordination and quality assurance monitoring of services
• Maintains required documentation in adherence to established standards.

Care Manager/Case Manager Job Requirements:

• Education: Bachelor's Degree in a Human Services or closely related area required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
• Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.
• Certification: Eligibility for long-term care functional screener certification is required
• Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Care Manager Job Schedule:

• Monday to Friday, 8:00 AM-4:30 PM
