Position Overview Family & Children’s Services is seeking a Youth Care Coordinator to provide targeted support services and Youth-centered Systems of Care (SOC). SOC coordinates mental health and community services with the needs of youth and families impacted by trauma by preventing out-of-home placements and encircling families with support and resources. Family & Children’s Services is Oklahoma’s leading provider of behavioral health care and family services. Care Coordinators work with children in their homes and communities while incorporating the entire family in treatment options. SOC focuses on youth strengths, age, cultural heritage, and wraparound services to strengthen healthy home, school, and family experiences. Compensation and Benefits Annual salary: $55,000. Up to 34 paid days off in the first year. Overflowing benefit package. Position Specific Duties & Responsibilities Conduct comprehensive assessments of clients' physical, psychological, social, and environmental needs. Implement individualized care plans in collaboration with clients, families, and other stakeholders that address unique needs and goals, promoting reintegration into the community through tailored support and skill-building activities. Coordinate and monitor the delivery of services to ensure timely and appropriate care. Advocate for clients' rights and assist them in accessing necessary resources and services. Provide emotional and practical support to clients and their families. Regularly evaluate clients' progress and adjust care plans as needed. Maintain accurate and up-to-date documentation of clients' care, progress, and outcomes. Facilitate communication and collaboration among multidisciplinary teams, including healthcare providers, social workers, and therapists. Participate in case conferences, team meetings, and other collaborative activities to ensure coordinated care. Educate clients and families about available services and resources and assist them in making informed decisions. Ensure compliance with all relevant laws, regulations, and standards of practice. Participate in continuing education, training, and professional development opportunities to stay current with best practices and trends in case management; complete certification renewals and all assigned trainings by the deadline. Qualifications Bachelor's Degree required; Bachelor's Degree in Social Science preferred. Experience in the field of mental health, criminal justice, or community-based social services required. Autonomy: Initiate and prioritize work, maintain self‑motivation, manage time independently, and ensure client care delivery with minimal oversight. Certifications & Licenses ODMHSAS Case Manager II Certification, completed within 60 days of eligibility. Wellness Coach Certification (required). Valid driver license with satisfactory driving record for travel to locations beyond the primary office or for client transportation. Other Information Safety Sensitive Job Classification This job is classified as a “safety-sensitive” position as defined by the Oklahoma Medical Marijuana and Patient Protection Act. Employees in this position will be subject to drug and alcohol testing, including random testing. Marijuana is included in the drug panel screening. Possession of a medical marijuana license will not excuse testing. Positive results may result in revocation of the job offer or dismissal per the Family & Children’s Services Drug Free Workplace Policy. Care Coordinator Role Focus Facilitate communication among treatment team, internal and external providers, clients, and families. Coordinate appointments, referrals, and services within the healthcare system. Fulfill transportation needs as applicable to the program. Ensure timely and appropriate care transitions. Provide support and education to clients and families regarding upcoming appointments. Obtain ROIs, make medical record requests, and ensure the treatment team receives up‑to‑date records. Assist nursing staff with medical record reconciliation as needed. May be required to work evening hours, weekends, and/or holidays. #J-18808-Ljbffr
