Responsibilities
- Deliver physical therapy services to residents in a skilled nursing facility, focusing on mobility, strength, balance, and functional independence
- Assess residents, develop individualized treatment plans, and monitor progress toward goals
- Implement evidence-based interventions and modify plans as needed; coordinate discharge planning with the care team
- Document evaluations, treatment plans, progress notes, and outcomes in resident records per facility policy
- Collaborate with an interdisciplinary team including nursing, therapy assistants, and physicians to optimize resident care
- Maintain safety, infection control, and patient-comfort standards during therapy sessions

Requirements
- Active Physical Therapist license or equivalent credential to practice physical therapy
- Ability to work in a skilled nursing facility environment
- Willingness to commit to a 13-week travel assignment
- Availability for a weekday day shift (08:00–16:30) on a 5x8 schedule
