As a Home Health RN, you will provide skilled nursing care to patients in their homes throughout Saluda and surrounding communities. A typical day includes traveling between patient homes, conducting visits, documenting in the EMR, coordinating with physicians and other clinicians, and educating patients and families on managing chronic conditions, medications, and safety in the home.

The role is largely field-based, with most time spent in patient homes, supplemented by remote documentation and communication with the care team. You will collaborate with an interdisciplinary group of professionals, including therapists, social workers, and home health aides, to support a coordinated approach to care. Patient caseloads focus primarily on adult and geriatric patients with chronic illnesses, post-acute needs following hospitalization, and rehabilitation after surgery or injury. Visit volumes are structured to allow thorough assessment, education, and documentation without sacrificing quality of care. Shifts typically follow daytime hours, with some flexibility based on patient needs and agency scheduling practices. Weekend or on-call rotations may be part of the role, depending on agency coverage requirements.
