Patient-Centered Clinical Execution: Lead individualized care journeys by executing complex medical plans with precision, ensuring every clinical decision prioritizes patient safety, comfort, and personal dignity.
Interdisciplinary Coordination: Serve as the central communication hub for the interdisciplinary team to facilitate seamless collaboration and ensure the delivery of holistic, well-integrated care.
Clinical Advocacy & Trend Monitoring: Drive optimal patient outcomes by proactively tracking health trends and documenting progress, enabling the medical team to provide timely, life-changing interventions.
Transition & Discharge Leadership: Empower patients and their families for life after discharge by leading comprehensive transition planning and providing the tools necessary for a successful recovery at home.
What minimum requirements you’ll need
Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date reqired. American Heart Association or American Red Cross accepted. Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date required. Education: Required professional licensure/certification AND 1 year of cumulative job specific experience required.
