In-Home Health - Nurse Practitioner or Physician Assistant (Part Time)

Role Part-Time Clinician (Nurse Practitioner or Physician Assistant) providing in-home health evaluations to improve patient outcomes. Responsibilities Conduct 45-minute in-home health assessments, including medical history review, physical evaluation, and optional diagnostic tests. Utilize an iPad and clinical workflow; no prescribing, ordering labs, or changing treatments required. Build trusted relationships with patients and connect them to appropriate care. Perform virtual visits as needed and travel within licensed areas. Deliver Diagnostic & Preventive Services like spirometry, bone density screening, etc. Qualifications & Skills Active, unrestricted licenses in designated areas. Board certification required; multi-state licenses preferred. Ability to work in diverse home environments with pets, smoke, or allergens. Key Duties Engage with patients in their homes to promote health, provide personalized evaluations, and support care bridging.
