Key Responsibilities

• Evaluate, treat, and develop patient care plans in accordance with Physical Therapy standards of practice for adult and geriatric populations
• Manage a caseload with an expected average of 4.5 patient visits per day
• Document patient care using point-of-service laptop and NetSmart EMR to meet regulatory and reimbursement requirements
• Participate in a rotating Saturday schedule approximately once every 6–8 weeks, with a compensatory weekday off in the same week
• Travel occasionally to other branch locations as needed; mileage reimbursed at the government-approved rate
• Adhere to facility infection control policies, including masking requirements for staff who decline flu and COVID vaccination during flu season

Required Qualifications

• Active Physical Therapy licensure in the State of Wisconsin (required)
• AHA BLS certification required
• Minimum of a Master's degree in Physical Therapy
• NPI number required; candidates without one must initiate the application process prior to start
• Must live outside a 50-mile radius of the facility to be eligible

Preferred Experience / Skills

• Prior home health experience strongly preferred
• Experience working with adult and geriatric populations across orthopedic, neurological, and functional mobility diagnoses
• Familiarity with NetSmart EMR or similar point-of-service documentation platforms
• Adaptable, resilient, and comfortable managing change in a dynamic care environment
