Occupational Therapist

About the Job

Accelerated Home Health Care is seeking a licensed and compassionate Occupational Therapist (OT) to provide in-home occupational therapy services to patients under physician orders and in coordination with the interdisciplinary care team.

Key Responsibilities:

Provide in-home occupational therapy services , including Start of Care (SOC) and revisit visits.
Complete patient evaluations and establish individualized treatment goals.
Implement evidence-based interventions to improve independence in activities of daily living.
Educate patients and caregivers on adaptive techniques, safety strategies, and use of equipment.
Accurately document evaluations, treatment notes, and progress reports.
Communicate patient status and progress with the care team.
Follow physician orders, care plans, and all applicable state, federal, and agency regulations.

Minimum Certification & Licensing Requirements:

Active Occupational Therapist (OT) license in the State of Michigan.
Completion of required state and agency onboarding/credentialing.
Up-to-date TB test.
Valid driver's license and reliable transportation.

Preferred Qualifications:

Home health experience preferred (not required).
Strong clinical judgment and communication skills.
Ability to work independently in a home-based setting.
Organized, reliable, and patient-focused.

Working Conditions:

Services provided in patients' homes ; travel between visits required.
Flexible scheduling, including daytime hours and occasional weekends.
Physical demands include standing, bending, lifting, and assisting patients as needed.
