Ambulatory Clinical Pharmacist - Patient Care Leader

Geisinger Wyoming Valley seeks an ambulatory clinical pharmacist to ensure safe, efficacious, and cost-effective medication therapy for patients in the ambulatory setting. You will work within a multidisciplinary team under a collaborative practice agreement, providing direct patient care via in-person or telephone interventions.
