Director of Skilled Nursing

Director of Nursing (DON) – Home Health BrightStar Care | Scottsdale, Arizona Full-Time | $100,000 annually. BrightStar Care is seeking a compassionate, experienced, and results-driven Skilled Care Director of Nursing (DON) to lead skilled clinical operations, quality assurance, staff development, and patient care for our team based throughout the Phoenix Metro Area. You will oversee clinical care delivery, support patients and families, develop and coach nursing and caregiving staff, and ensure our team consistently delivers the BrightStar Care promise: A Higher Standard of Care. The DON will work closely with leadership, physicians, healthcare providers, families, community partners, and field staff to ensure exceptional patient outcomes, regulatory compliance, and an outstanding patient experience. Key Responsibilities Clinical Leadership & Patient Care Provide leadership and oversight of clinical care delivery and quality assurance. Conduct initial assessments, reassessments, supervisory visits, and other clinical visits as needed. Develop and oversee individualized patient plans of care. Provide hands‑on nursing and field support when operational or patient needs require it. Support staffing coverage and assist in the field when necessary. Participate in on‑call responsibilities as needed. Communicate effectively with patients, families, physicians, healthcare providers, and community partners to promote continuity and quality of care. Quality Assurance & Compliance Review clinical documentation, progress notes, care summaries, and patient records for accuracy and compliance. Ensure compliance with BrightStar Care standards and applicable federal and state home health regulations. Support Joint Commission readiness and compliance. Maintain documentation and records required for Joint Commission surveys. Serve as the primary clinical contact during surveys when applicable. Conduct monthly client chart audits using approved company tools. Maintain clinical records, reports, statistics, and other documentation required for quality evaluation and reporting. Identify opportunities for improvement and implement corrective actions when needed. Staff Development & Supervision Supervise, educate, coach, and evaluate nursing personnel and caregivers. Conduct ongoing skills evaluations and competency reviews. Ensure new nursing employees successfully complete clinical orientation and competency requirements. Plan and implement orientation, in‑service education, and continuing education programs. Conduct annual performance evaluations for nursing staff. Promote a culture of accountability, teamwork, professionalism, compassion, and continuous improvement. Help develop organizational clinical goals, policies, and procedures. Business & Community Relationships Promote home health referrals and help develop strong relationships with physicians, hospitals, healthcare organizations, and community partners. Represent BrightStar Care professionally within the healthcare community. Work collaboratively with the leadership team to support growth while maintaining exceptional clinical quality and patient outcomes. What We're Looking For Required: Active, unencumbered Arizona Registered Nurse (RN) license. Minimum of four years of nursing experience. Strong knowledge of home health regulations and clinical best practices. Excellent communication, organization, leadership, multitasking, and problem‑solving skills. Valid driver's license and reliable transportation. Current auto insurance. Current BLS/CPR certification. Updated TB test or chest X‑ray. Ability to travel throughout the service area and provide field support as needed. Ability to stand, walk, bend, kneel, squat, reach, push, and pull as required. Ability to lift or transfer up to 50 pounds. Preferred: At least one year of nursing leadership or clinical supervisory experience. Previous home health leadership experience.
