Position Highlights Skilled Nursing Facility / Post-Acute Rehab 13-Week Travel Contract Full-Time Schedule Adult & Geriatric Patient Population Collaborative Rehab Team Monticello, Florida Responsibilities Perform comprehensive occupational therapy evaluations and reassessments. Develop individualized treatment plans focused on improving ADLs and functional independence. Treat orthopedic, neurological, post-surgical, and medically complex patients. Provide ADL retraining, upper extremity rehabilitation, transfer training, cognitive interventions, and adaptive equipment education. Collaborate with nursing staff, physicians, PT, SLP, and case management. Educate patients and caregivers on safety strategies, home modifications, and discharge planning.
