Hospice Patient Care Manager

We are currently searching for a Patient Care Manager (PCM) . Your primary focus will be to oversee & lead the clinical team who is supporting individuals and families facing the serious illness challenges that arise at the end of life. • Lead and model expected behaviors with clinical with reference to our Care model, Signature Care
• Oversee Interdisciplinary Team (IDT).
• Performs routine education on infection control, assessments, continuity of care, and compliance standards.
• Coordinate field visits with the nursing team and as needed by the patient plan of care in an effort to support the patients and their families.  Be willing to fill gaps with patient visits from time-to-time if needed.
• Understand the hospice philosophy, pain management, and the needs of the terminally ill and be able to convey that in a language the clinical team, as well as the patient and family understand.
• Demonstrate excellent interpersonal and communication skills.

Qualifications:

• Degree in Nursing - ADN, BSN, MSN.
• Valid Texas RN License.
• Valid BLS.
• Minimum 2 years’ experience in the hospice setting.
• Previous Patient Care Manager experience preferred.
• Ability to pass background check and pre-employment drug screen.
