In-Home Nurse Practitioner or Physician Assistant Full Time - Columbus

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. This role involves engaging with people in the comfort of their own homes and helping bridge gaps in care. You’ll play a critical role in building trusted relationships to make people healthier, delivering flexible, comprehensive and personalized health evaluations right to people’s front doors. Responsibilities Perform in-home health evaluations, including a medication and medical history review, a physical examination, and documentation of findings. Conduct additional diagnostic tests (e.g., diabetic eye exams, spirometry) when ordered by the person’s health plan, as part of the evaluation. Utilize an iPad and a clinical workflow to support each evaluation. The role does not require prescribing medications, ordering lab tests, or altering a patient’s current treatment regimen unless directed by the health plan. Engage with health plan members to connect them with appropriate care and resources, aiming to improve health outcomes. Be flexible to travel locally and within licensed states, as needed, and conduct virtual visits as required. Provide all Signify Health services as indicated, including Diagnostic & Preventive Services (e.g., spirometry, DEE, bone density screening, etc.). Align with Signify Health values and collaborate with a national network of clinicians to deliver high-quality care. Qualifications Active, unrestricted license(s) in the coverage area(s) Board certification (required) Multi-state licenses (preferred) Compensation and Schedule Anticipated Weekly Hours: 40 Time Type: Full time Pay Range: $87,035.00 - $187,460.00 Base salary is subject to factors such as experience, education, geography and other factors; this range represents base compensation for positions within the job grade. Environments may vary and can include households with pets, secondhand smoke and other potential airborne allergens or irritants. Clinicians must be able to perform duties safely in home settings.
