Experienced Nurse Practitioners are invited to deliver direct patient care through home health risk assessments for Medicare Advantage health plan members. This opportunity provides you with control over your schedule, minimal administrative burden, and the chance to serve communities throughout Wise County.

Schedule:

• Flexible shifts available any day of the week (Monday through Sunday)
• Design your own schedule
• Minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage members in varied locations across Wise County
• Complete visits averaging approximately 45 minutes, which may include annual health evaluations or post-discharge follow-ups
• Review past and current medical histories, identify/resolve gaps in care, and provide recommendations to optimize patient health
• Educate members on their health conditions and medications
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Valid Nurse Practitioner license in Virginia
• Active CPR certification (BLS required)
• Board certification via AANP or ANCC
• At least 6 months of recent NP experience preferred
• MSN degree from an accredited nursing program

This position offers a highly competitive rate, paying $80$90 per completed evaluation, along with a mileage stipend and compensation for administrative time.
