Details Home health setting covering a territory in South Marin County, including Sausalito, Tiburon, Belvedere, and Mill Valley Day shift schedule, 5x8-hour shifts from 8:00 AM to 5:00 PM Caseload of approximately 5 visits/points per day Uses Epic EMR with OASIS documentation required Requirements Current CA PT License required (license required if applicable) BLS certification required Valid driver's license and auto insurance required Epic EMR experience required OASIS documentation experience required Minimum 2 years of experience required Prior travel experience required Strong critical thinking, independent judgment, and problem-solving skills required Effective verbal and written communication and interpersonal skills required Additional Information Responsibilities include patient assessment, treatment planning, and delivering therapy interventions across orthopedic, neurologic, and cardiopulmonary conditions in patients' homes Skills applied may include gait training, therapeutic exercise, wound/skin integrity assessment, and use of adaptive or assistive devices Collaborates with interdisciplinary care team members to develop and adjust treatment plans Provides patient and family education on home exercise programs and safety Maximum RTO of 7 days per contract, with only 1 holiday request approved per contract Local candidates accepted within a 50-mile radius at the local rate Returning staff/travelers must have a 1-year separation before being eligible to return as a traveler American Traveler Job ID P-729825.
