Physical Therapy Assistant — Home Health, travel assignment in Monterey, CA. Start date 06/29/2026 for 13 weeks. Weekly pay range $1,297–$1,330 per week with guaranteed 38 hours. Shift: Days. Responsibilities
- Provide skilled physical therapy assistance under the supervision of a licensed PT to SNF residents, implementing and adapting treatment plans.
- Deliver therapeutic interventions such as gait training, therapeutic exercises, functional mobility training, ADL training, and activity modification as directed.
- Monitor patient progress, document outcomes, and adjust plans in collaboration with the supervising PT.
- Conduct accurate charting and data entry in NetHealth (Rehab Optima) to reflect services provided, responses to therapy, and discharge planning notes.
- Collaborate with the interdisciplinary team (nursing, OT, SLP, social services) to coordinate care and optimize resident outcomes.
- Ensure patient safety, apply proper infection control, maintain equipment, and communicate changes in status promptly.

Requirements
- Active California Physical Therapy Assistant license
- Minimum 1 year of SNF experience
- Knowledge of NetHealth (Rehab Optima) or equivalent rehab documentation system
- Ability to work days in a Skilled Nursing Facility setting
- Strong communication and teamwork skills; ability to follow treatment plans under supervision
