CARE MANAGER RN

Overview Riverside Healthcare is seeking a dedicated and compassionate Care Manager Registered Nurse to join our dynamic team. This role is integral in coordinating patient care, improving health outcomes, and ensuring a seamless transition through various levels of care. The ideal candidate will possess a blend of clinical expertise, strong organizational skills, and the ability to advocate for patients and their families. Additionally, this position requires flexibility with assignments within professional scope/duties/licensure. Essential Duties Patient Care Coordination: Develop, implement, and manage individualized care plans for patients with complex health needs. Collaborate with multidisciplinary teams to ensure comprehensive and effective care. Completes management functions including patient/family education. Refers reviewed cases to a Physician Advisor as needed. Assessment and Planning: Conduct thorough assessments of patients medical, psychological, and social needs. Create and adjust care plans based on ongoing evaluations and patient progress. Patient Advocacy: Serve as a primary contact for patients and their families. Provide education on health management and resources, ensuring patients understand their care plans and treatment options. Case Management: Monitor and evaluate patient care and outcomes, coordinating with healthcare providers to address any issues or changes in patient condition. Ensure timely follow-up and continuity of care. Documentation: Maintain accurate and up-to-date records of patient interactions, care plans, and outcomes. Ensure compliance with regulatory requirements and organizational policies. Resource Management: Identify and facilitate access to community resources and support services for patients. Assist in managing care costs and optimizing the use of healthcare resources. Quality Improvement: Participate in quality improvement initiatives to enhance patient care and optimize processes within the care management program. Non-essential Duties Administrative Support: Assist with administrative duties related to care management, such as preparing reports and attending meetings. Training and Development: Contribute to the training and orientation of new staff members or interns as needed. Responsibilities Required Experience Minimum of 3-5 years of experience in a clinical setting. Preferred Experience Experience in managing complex cases and working with diverse patient populations is highly desirable. Preferred previous experience with case management and utilization management. Strong clinical assessment and critical thinking skills. Excellent communication, organizational, and interpersonal skills. Proficiency with electronic medical records (EMR) and care management software. Ability to work independently and as part of a team. Strong commitment to patient-centered care and continuous professional development. Required Licensure/Education Current Registered Nurse (RN) license in the state of Illinois. New Graduate RNs starting on or after 1/1/25 must earn Bachelors of Science in Nursing (BSN) within 3 years of start date. Preferred Education Bachelors Degree in Nursing (BSN) or higher from an accredited institution. Certification in Care Management (CCM) or similar certification is preferred.
