Quality Improvement Specialist

To support a growing healthcare initiative, the remote Quality Improvement Specialist will manage patient and provider panels, deliver performance insights, and drive improvements in HEDIS and Medicare Stars measures through education and collaboration. Key responsibilities Engage with providers to educate on quality measures, documentation standards, and performance improvement strategies Manage a patient panel by conducting outreach to close care gaps and enhance patient well-being Collaborate with cross-functional teams to ensure compliance and support quality initiatives Required qualifications 1-2 years of experience in quality improvement, population health, or care coordination Patient-facing outreach experience Understanding of quality workflows and documentation requirements EMR proficiency and familiarity with healthcare technology tools Intermediate skills with MS Office Suite, including Outlook and Teams
