Position Summary:
At Children's Aid, the School-Based Health Center (SBHC) Lead Nurse Practitioner serves as both a clinical provider and site leader within a network of school-based health centers serving students from Pre-K3 through high school. In collaboration with a supervising physician, the Lead Nurse Practitioner provides primary, acute, and reproductive healthcare services to students while supporting the overall health and wellness goals of the school community. The incumbent also provides day-to-day leadership across the SBHC site, helping ensure high-quality patient care and compliance with New York State Department of Health (NYS DOH), New York City Department of Health and Mental Hygiene (NYC DOHMH), and Children's Aid policies and procedures.
Working closely with the SBHC Program Manager, the Lead Nurse Practitioner supports medical staff oversight, contributes to performance evaluations, and helps maintain efficient clinical operations and workflows. This role also partners with the Children's Aid Community School Director, school leadership, and other stakeholders to support integrated student services and represent the Health and Wellness Division in meetings and school-based initiatives. As part of a collaborative and dynamic medical team, the Lead Nurse Practitioner plays a key role in advancing accessible, student-centered healthcare services within the school setting.
Essential Duties:
Clinical Care & Patient Services
- Provide comprehensive primary, acute, and reproductive healthcare services to pediatric patients within the School-Based Health Center (SBHC) program in collaboration with the supervising physician.
- Obtain and document patient medical, developmental, and psychosocial histories and conduct physical assessments and screenings in accordance with clinical standards and NYSDOH guidelines.
- Assess patient health status, identify normal and abnormal findings, and recognize deviations in growth and development to support appropriate treatment planning and referrals.
- Order, interpret, and follow up on routine diagnostic tests, screenings, referrals, emergency visits, clinic visits, and hospitalizations as appropriate.
- Provide treatment for minor illnesses and injuries and seek collaborative physician consultation when clinically appropriate.
- Administer routine immunizations in accordance with NYSDOH standards and maintain site eligibility within the Vaccines for Children Program.
- Provide outpatient follow-up care and guidance to patients and families, including telephone triage and support related to ongoing medical concerns.
- Maintain accurate, timely, and legible medical documentation in accordance with organizational, regulatory, and Medicaid standards.
- Demonstrate clinical competency in providing appropriate age care to the populations served across the SBHC network.
Leadership, Operations, & Site Management
- Provide day-to-day clinical and operational leadership for the SBHC site and serve as a resource to staff involved in patient care and service delivery.
- Collaborate with the SBHC Program Manager, administrative staff, and supervising physician to support efficient site operations and high-quality patient care.
- Supervise medical clerical staff and contribute to performance management and evaluation processes.
- Support site productivity goals and ensure effective coordination of daily clinical operations and workflows.
- Participate in the review, implementation, and continuous improvement of medical protocols, workflows, and quality assurance practices.
- Promote and support the organization's Mission, Vision, and Core Values through daily practice and leadership.
Compliance, Quality Assurance, & Professional Practice
- Ensure compliance with NYS DOH, NYC DOHMH, HIPAA, HITECH, OSHA, infection control, and organizational policies and procedures.
- Partner with the SBHC Program Manager to support ongoing quality assurance activities, emergency preparedness requirements, environmental surveys, audits, and regulatory reporting.
- Maintain required statistical reporting and documentation to support compliance and operational oversight.
- Participate in after-hours on-call coverage on a rotating basis.
- Maintain current clinical knowledge and participate in ongoing professional development and training opportunities to support evidence-based patient care.
- Ensure all patients, families, visitors, and colleagues are treated with dignity, professionalism, and respect.

Minimum Qualifications:
- Master of Science in Nursing from an accredited university required.
- Certified as a Pediatric Nurse Practitioner and Board Certified via the National Association of Pediatric Nurse Associates and Practitioners, or the American Nursing Association.
- Current license to practice professional nursing and as a nurse practitioner in the State of New York.
- Continuing Education Unit Accreditation Maintenance.
