Hospice RN Case Manager

Optimal Care seeks a Hospice RN Case Manager to provide compassionate end-of-life care in patients’ homes and facilities. In this role, you’ll complete admissions, assessments, and individualized care plans; manage symptoms; and administer medications and treatments. You will coordinate with an interdisciplinary team, educate families, and ensure patient comfort and dignity. The position includes documentation, care-plan updates, and participation in an on-call rotation to support patients and families during urgent needs.
