Physical Therapist (PT)

Job Summary

We are seeking a dedicated Physical Therapist to provide high-quality home health care to patients. The ideal candidate will have a strong background in physical therapy, excellent communication skills, and the ability to work in various home environments.

Job Details

Unit: Home Health
Shift: Monday to Friday, 8:30 AM to 5:00 PM
Shift Length: 8 hours
Guaranteed Hours: 40 hours per week
Required Experience: Minimum of 2 years in a healthcare setting, with home health experience preferred

Responsibilities

Deliver physical therapy treatments in patients' homes, assisted living facilities, senior apartments, and board and care homes.
Manage a caseload of 3 to 5 patients per day, including 2 Start of Care visits and 3 to 4 evaluations.
Complete all required documentation and case management within the shift hours.
Work in collaboration with a RN Manager and support from a Regional Rehab Leader/Educator.
Supervise up to 2 Physical Therapy Assistants (PTAs) or Certified Occupational Therapy Assistants (COTAs).

Qualifications

Current California Physical Therapist license is required.
Basic Life Support (BLS) certification from the American Heart Association is mandatory.
Experience with HomeCare Home Base (HCHB) EMR is preferred.
Knowledge of OASIS assessments is required.
Must possess a valid driver's license and proof of automobile insurance.
Ability to work effectively in diverse home environments.

Additional Information

Orientation will include 3 weeks of training with both online learning and field precepting.
A weekend shift is required once per month, though this is flexible.
Holiday availability is expected, with some flexibility for time off requests.
Professional attire is required; scrubs are recommended. Closed-toe shoes are mandatory.
