Therapy – Physical Therapist (PT)

Location: Tampa, FL | Home Health

Schedule / Job Type: Contract | Days | Full-Time (5×8 Hours, 8:00–17:00)

Key Responsibilities:

• Conduct skilled home health visits, with an expected 6–8 patient visits per day
• Perform comprehensive patient assessments including cardiopulmonary, neurological, and orthopedic evaluations
• Develop and implement Home Exercise Programs and provide patient and family education
• Utilize and assess equipment and devices including gait belts, rolling walkers, sit-to-stand lifts, sliding boards, wheelchairs, and upper extremity prosthetics
• Complete accurate clinical documentation including OASIS and billing using designated scheduling and timesheet systems
• Identify and report signs and symptoms of physical, sexual, and psychological abuse; ensure safety and injury prevention for all patients

Required Qualifications:

• Active Florida or Compact Physical Therapist license required at time of submission
• BLS certification required
• Minimum 1 year of clinical experience required
• Home health experience required
• Valid driver's license and personal vehicle required to perform home visits
• Must be COVID-19 vaccinated or have an exemption form on file; flu shot or declination required (mandatory masking applies in either case)
• Locals are accepted — candidates must reside more than 100 miles from the facility to qualify as a traveler; local candidates are welcome at the same rate

Preferred Experience / Skills:

• OASIS training preferred
• Cerner EMR experience preferred
• Experience treating a diverse patient population including ortho, neuro, cardiac, COPD, CHF, CVA, Parkinson's, trauma, and post-surgical patients
