Qualifications and requirements:

• Minimum 18 years old
• Ensure the schedule is completed in such a way as to retain all clients, hours, and caregivers
• Communicate staffing and availability needs to the appropriate supervisor
• Positive attitude, great communication skills, and ability to work independently
• A solid personal character with an empathetic personality
• Must be able to pass both a criminal background check and drug screen

Overview:

The Care Transition Coordinator is an experienced field marketer responsible for company growth. As a Care Transition Coordinator, you will be responsible for developing our Home Health/Home Care presence within supportive livings and home and community-based residences. Responsible for generating new referrals by building relationships with area hospitals, healthcare systems, case managers, and other community resources.

Essential duties and responsibilities:

• Participate in short and long-range planning to achieve company goals and objectives.
• Responsible for complete referral process, including identifying home health needs, verification of benefits, and coordination of care.
• Cultivate new and existing relationships resulting in company growth.
• Represent the agency in a professional, prepared, and caring manner.
• Prepare weekly reports on all marketing activities.
• Initiate and expand referral network.
• Comply with all aspects of the company's compliance and HIPAA regulations.
• Performs other duties as assigned.

Education and experience:

• Bachelor's degree preferred
• One year of home health/home care, marketing, or case management experience or a combination of each
• Must possess a valid driver's license
• This professional will uphold the policies and procedures established by Purpose Home Health. They must also demonstrate excellent oral, written, and active listening skills.
• Ability to work independently, maintain confidentiality, and meet assigned deadlines.
• Demonstrate sound judgment and good decision-making skills.
