Medical Social Worker - Case Management

We are seeking a compassionate and experienced Medical Social Worker to join our acute care case management team. The ideal candidate will have a strong background in discharge planning and case management, with the ability to work effectively in a fast-paced environment.

Responsibilities

• Conduct comprehensive biopyschosocial assessments to evaluate patient needs.
• Develop and implement effective discharge plans tailored to individual patient circumstances.
• Coordinate post-acute care services to ensure continuity of care.
• Collaborate with multidisciplinary teams, including medical staff and case managers, to achieve patient care goals.
• Provide crisis intervention and guidance to patients and their families, facilitating access to community resources.
• Maintain thorough documentation of patient interactions, care plans, and progress notes.

Required Experience / Certifications / Licensure

• Minimum of 2 years of relevant experience in medical social work, specifically in acute care settings.
• Must possess a valid state license as a Medical Social Worker (MSW).
• Experience with Epic or similar electronic health records systems is preferred.
• Knowledge and understanding of healthcare regulations, including Medicare and Medicaid.
• Bilingual candidates are a plus.

Schedule

This is a Day 5x8-Hour position, with shifts from 08:00 AM to 04:30 PM. There may be a weekend requirement based on departmental needs.
