Behavioral Health Care Manager

Maintain a caseload of up to 50 members and conduct monthly in-person visits Manage ECM members with behavioral health needs, primarily Tier 1 and Tier 2 members Conduct comprehensive biopsychosocial assessments by telephone and in person Develop therapeutic relationships that promote engagement, self-management, and improved health outcomes Utilize Motivational Interviewing, Behavioral Activation, solution-focused techniques, and brief therapeutic interventions Provide crisis assessment and facilitate warm handoffs to crisis response resources Coordinate behavioral health, medical, and social services for integrated whole-person care Support members through hospital admissions, discharges, and other care transitions Conduct face-to-face visits based on member risk stratification and health plan requirements Maintain member engagement through telephone outreach, home visits, and community-based encounters Collaborate with providers, hospitals, health plans, and community organizations Participate in ICT meetings, Systematic Case Reviews, and case conferences Develop and maintain member-centered shared care plans Coordinate referrals for behavioral health, substance use treatment, housing, transportation, food insecurity, and other social needs Monitor progress toward behavioral health and wellness goals Track assessments, screenings, authorizations, and documentation in the care management platform Ensure timely documentation meeting Titanium Healthcare, MCP, and DHCS requirements Educate members and families about behavioral health conditions, treatment options, medications, and coping strategies Provide behavioral health consultation to interdisciplinary team members Assist with staff education on behavioral health best practices and evidence-based care Promote integrated behavioral and physical healthcare Maintain compliance with Titanium Healthcare policies, DHCS regulations, ECM requirements, and MCP standards Support quality improvement initiatives, audits, quality reviews, and performance improvement initiatives Complete required documentation and other assigned duties and projects Requirements Master's degree in social work, counseling, psychology, or another related behavioral health field Minimum of 2+ years of behavioral health, care management, case management, or community-based experience Current and valid driver's license with a clean driving record Reliable transportation and proof of auto insurance Secure, distraction-free home workspace with reliable internet connection Strong knowledge of behavioral health assessment, diagnosis, and treatment planning Understanding of Serious Mental Illness (SMI), Substance Use Disorders (SUD), trauma-informed care, and recovery-oriented practices Knowledge of Enhanced Care Management (ECM), CalAIM, and population health principles Experience utilizing Motivational Interviewing and other evidence-based engagement techniques Excellent crisis assessment and de-escalation skills Strong care coordination and interdisciplinary collaboration skills Ability to navigate complex healthcare and community resource systems Excellent documentation, organizational, and prioritization skills Proficiency with electronic health record systems Fluent in English, written and verbal Ability to lift, carry, push, or pull up to 20-25 pounds Ability to climb stairs and navigate uneven terrain Ability to conduct visits in non-traditional environments and travel throughout the community Ability to operate a vehicle Competence with computers, email, virtual platforms, and Microsoft Office-based programs Nice to have: Experience in ECM, CalAIM, or managed care Nice to have: Current unrestricted CA LCSW, ACSW, LMFT, AMFT, LPCC, APCC, or equivalent licensure/registration Nice to have: Experience with SMI, SUD, homelessness, or high-risk populations Nice to have: Experience with eClinicalWorks (eCW) Bilingual Spanish preferred Demonstrates expertise in behavioral health assessment, crisis intervention, and care coordination, with a strong focus on integrated care and member engagement.
