About the Role:

As a LPN at Advocate Healthcare of East Boston, you will play a vital role in delivering high-quality patient care. You will work collaboratively with our multidisciplinary team to provide personalized care to our residents, ensuring their physical, emotional, and spiritual needs are met.

Responsibilities:

• Provide direct patient care to residents, including administering medications, performing vital signs, and providing wound care
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Coordinate with other healthcare professionals to ensure seamless transitions of care
• Maintain accurate and detailed records of patient care and treatment plans
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:

• Current LPN license in the state of Massachusetts

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment
