Every day, we serve approximately 1,500 patients living with advanced illness, wherever they call home, be it a private residence, a long-term care or assisted living facility, a hospital, or in one of our nine inpatient care centers. And most importantly, as the only non-profit hospice provider in the region, we never turn anyone away due to an inability to pay. Position Summary The Care Navigation Center (CNC) Coordinator PRN role is responsible for processing referrals into Community Hospice and Palliative Care, PACE, and Home Health programs. This includes completing intake processes, entering data, verifying insurance, scheduling assessments, and coordinating with referral sources, patients, and families. The role ensures seamless and efficient transitions into care and delivers excellent customer service. Essential Functions Serve as the primary contact for inbound and outbound customer service. Provide support via phone, fax, or email to internal and external stakeholders. Document all communications and referral details accurately in electronic systems. Facilitate referrals with a focus on customer service and efficiency. Respond to inquiries related to hospice, palliative, PACE, and home health services. Coordinate service initiation with admissions staff, clinicians, and external partners. Schedule assessment visits promptly and cost-effectively. Accurately input data into Electronic Medical Records (EMR) and relay referral details. Take ownership of referrals and ensure timely follow-up. Explain services to patients and families by phone or in person. Effectively multitask in high-volume environments. Seek nurse guidance when managing complex clinical intake cases. Maintain strict confidentiality in accordance with HIPAA regulations. Meet performance standards related to productivity, accuracy, and timeliness. May mentor or support less experienced team members. Be available for overtime and weekend work as needed. Perform additional duties as assigned. Qualifications Knowledge and Critical Skills Strong customer service and call center capabilities. Proficiency in data entry and computer systems. Effective communication, time management, and organizational skills. Ability to interact professionally and empathetically with others. Understanding of healthcare referrals and service coordination. Experience At least five (5) years of customer service experience. At least five (5) years of experience in a call center environment. Healthcare environment experience is preferred. License / Certification None required Education High school diploma required. Medical certificate (e.g., CNA, Medical Coding) or associate degree or higher preferred.
