Job Description Reports to the team coordinator Assumes responsibility and accountability for a designated group of patients and accurately provides Physical Therapy (PT) evaluations; develops and implements PT treatment plans; provides education and information to patients and caregivers; participates in required discharge planning and completes all required documentation according to policy. Supervises and assumes responsibility for provided Licensed Physical Therapy Assistants. May be assigned other duties such as clinical instructor for student affiliations, performance improvement, and program development.
