This full-time, onsite position invites you to play a crucial role in providing holistic and compassionate care to patients managing chronic pain. Work Environment

This role is set in the dynamic environment of concierge medicine and house calls, where you will have the opportunity to engage with patients within their own homes, enhancing comfort and accessibility to care. Working hours are typically from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Additionally, participation in community outreach events during weekends may be required, allowing you to advocate for health equity directly within the community.

Key Responsibilities

• Conduct thorough health assessments and screenings for patients, evaluating their medical history and current health status to formulate individualized care plans.
• Diagnose and manage both acute and chronic pain conditions, utilizing evidence-based practices to enhance patients’ quality of life through effective treatment and preventive care.
• Develop and implement community health education programs in collaboration with local organizations to raise awareness about pain management options and related health issues.
• Maintain detailed and accurate medical records, ensuring compliance with all regulatory standards and facilitating effective communication within the healthcare team.
• Provide culturally competent care, advocating for the rights of patients and addressing barriers to healthcare accessibility.
• Coordinate referrals to specialists and follow up to ensure continuity of care and optimal health outcomes for each patient.
• Engage in community outreach, including health fairs and screenings, to promote health awareness and educate the public about available healthcare resources.
• Stay current with ongoing trends and advancements in pain management and community health initiatives through continuous education and professional development.

In this role, you will primarily serve a diverse patient population that encompasses individuals facing chronic pain and other related health issues. Your focus will be on providing compassionate care to those who may lack access to quality healthcare services. Qualifications

• Current certification as a Nurse Practitioner in the state of New Jersey.
• An active state nursing license is required.
• A Bachelor’s Degree in Nursing, with a preference for candidates who hold a Master’s Degree from an accredited program.
• Minimum of three years of clinical experience as a Nurse Practitioner, particularly in pain management or community health settings.

Preferred Experience

• Experience working in concierge medicine or home healthcare settings is highly desirable.
• Strong interpersonal and communication skills are essential for effectively interacting with diverse populations and collaborating with various stakeholders.
• A commitment to social justice and health equity, demonstrating skills in advocacy for vulnerable communities.

Compensation and Benefits

The compensation package for this role includes a competitive salary range of $120,000 to $130,000 annually, commensurate with qualifications and experience.
