How you’ll make an impact in this role

• Assess patients with chronic or acute heart failure through physical exams, symptom review, health history evaluation and monitor vital signs and weight trends.
• Educate patients and families on heart failure management, medication adherence, and lifestyle changes
• Coordinate care plans with providers, including titrating medications per protocols.
• Manage incoming calls to assess symptoms, adjust care or refer to a physician.
• Act as a liaison between physicians and patients, facilitating follow-up appointments and diagnostic tests.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• One or more of the following required:
  • Licensed Registered Nurse credentialed from the Tennessee Board of Nursing OR current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date. License eligible may be accepted.

Education:

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of
  hire.
