Currently hiring for Home Health PT for Stockton, CA with multiple openings.

We are looking for high quality, full-time & part-time Physical Therapists (PT) to join the premier home health care team!
Flexible Schedule - schedule your own visits
We are offering sign on bonuses of up to $10,000 for full-time Physical Therapists.

Prohealth is a locally owned Home Health, Hospice & Palliative Care agency with 4 locations serving over 12 counties in Northern California. We maintain an average daily census of over 1800 patients. Being an industry leader for over 15 years, we pride ourselves in providing exceptional clinical care with compassion to our patients. Our patient focused care model provides personalized care, ranging from nursing, physical therapy, occupational therapy, speech therapy, and even registered dietitian.
Prohealth Home Care is seeking a compassionate and dedicated Home Health Physical Therapist (PT) who will aim to make a positive difference in our community. Our focus is to provide skilled and dignified care in the comforts of their home while providing emotional support to our patients and their families.

We are offering sign on bonuses of up to $10,000 for full-time Physical Therapists

Job Responsibilities:

Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment
Assesses for muscle strength, mobility, gait, ROM -- potential for rebab
Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences
Identifies patient and family/caregiver needs for other home health services and refers as necessary

Qualifications:

Valid PT License to Practice in CA
Current BLS/CPR certification
Valid CA Driver’s license
Minimum of one year Physical Therapy experience is (required/preferred)
